What do we offer?

The RHN helps people living with diabetes, chronic obstructive pulmonary disease (COPD) or heart failure to connect with the range of primary health care and community services they need to optimally manage their condition.

The RHN does not replace your doctor or other health provider. The RHN acts as a resource, working with you to help you achieve your health goals. For example, they can link you with the services, information and other supports you need to help you follow your chronic condition management plan.

With your permission, the RHN may also communicate with your healthcare providers to improve coordination of your care.

After your goals have been achieved, the RHN will follow up with you at set periods. You can reapply for further navigation support if needed.

Community availability

The Regional Health Navigator provides support for people living in the Mildura, Buloke and Swan Hill LGAs.

People living in border towns who access services in Mildura, Buloke and Swan Hill LGAs are also eligible for the navigation service.

How to access this service

The service is available, free of charge, for people who:

  • have been diagnosed with:
    • diabetes,
    • chronic obstructive pulmonary disease (COPD) or
    • chronic heart failure
