Social Support Programs

What do we offer?

Social support programs are held regularly at community venues across the MTHCS catchment area. We have a team of experienced staff who coordinate activities, social gatherings and friendly conversations.

Activities may include outings, craft, games, reminiscing and attending community events of interest. Group members are encouraged to contribute to the planning of upcoming activities.

Weekly programs, also known as Planned Activity Groups (PAGs), provide opportunities for people to participate in activities and connect with others.

MTHCS also coordinates a range of monthly and regular social programs, including:

• Ouyen Meet & Eat

Friendly Visiting

Not everyone likes to attend group-based social support programs. Social support can also be provided regularly in a customer’s own home.

The Friendly Visiting Program matches volunteers with people living at home who would like someone to chat with, need assistance with pets or outings, or would like support to see a health professional.

Who is eligible?

You may need to be assessed through My Aged Care.

Who are the friendly visitors?

Friendly visitors are local volunteers who spend time visiting people who live at home and may be house-bound because of mobility or health issues.

Community availability

Social support programs are available in Culgoa, Murrayville, Ouyen, Patchewollock, Sea Lake, Speed, Underbool and Woomelang. Please contact MTHCS for current program dates, times and locations.

How to access this service?

To accessing social support programs, you may need to be assessed through My Aged Care or undertake a HACC assessment to determine your eligibility.

If you are under 65 or have an NDIS package, please call MTHCS on (03) 5092 1111. Our staff can assist you on your journey to access our social support programs.
