Self-Management Program
The aim of the Diabetes Self-Management Program is to identify and improve the health of people with diabetes.
The program supports people diagnosed with type 2 diabetes to access greater knowledge about diabetes, develop diabetes self-care skills, and have regular diabetes health checks.
