Palliation

 

Anyone is eligible to receive community nursing services if there is an identified need. For example:

  • A frail aged person or someone with a disability requiring support to remain at home.

  • Recent discharge from hospital requiring short term care

  • Carers requiring support

  • People with chronic and/or incurable health conditions

The team provides outreach to all areas within the Mallee Track area.

 

Services can include:

  • Medication management

  • Wound management

  • Chronic ulcers

  • Post-surgery

  • Burns

  • Pressure sores

  • Health monitoring

  • Carer support

  • Palliative care

  • Assessment and referral to other community services

  • Palliative Care (Assistance to maintain comfort and dignity when experiencing an incurable condition)

  • General advice and information on appropriate community supports

 

Fees

The government sets the fees that can be charged for HACC eligible clients. For people who fall outside this category, fees are assessed based on the client’s income. MTH&CS ensures any fees charged are kept to a minimum. Department of Veteran Affairs clients may be exempt from fees. Please contact the community nursing service for current fees and charges, discuss any concerns you may have about fees with your nurse.
